In this particular stipulation, a scaffold is set up above the ring or in the stage area, featuring a walkway (usually made of metal or sometimes wood) elevated several meters off the ground.

The match occurs exclusively on the walkway and can end in two different ways, depending on the chosen method: the first, and most commonly used, involves throwing the opponent off the walkway (which makes the match particularly dangerous due to the height of the structure). The second method involves retrieving a flag placed on the opponent's side of the walkway (similar to a Flag Match) and bringing it back to one's "base," or side of the walkway, before the opponent does the same.

In some cases, particularly in ECW, to make the fall from the walkway even more painful, various objects, such as stacked wooden tables, were placed below. If barbed wire was positioned beneath the walkway, it was referred to as a Circus Death Match.

The Scaffold Match does not allow for pinfalls, submissions, count-outs, or disqualifications (DQ) and is contested only in 1 vs 1 or tag team formats.

A variant of this stipulation is the Elevation X Match, used in TNA, which differs from the Scaffold Match only in the shape of the walkway: in this version, the match takes place on two intersecting walkways forming an "X", and it is won by throwing the opponent off.
